Pressure ulcers — also called bedsores, pressure sores, or pressure injuries — are wounds that develop when sustained pressure cuts off blood flow to an area of skin. They most commonly affect people who are bedridden, use wheelchairs, or have limited mobility. Left untreated, they can progress from mild skin discoloration to life-threatening infections involving bone and muscle.

The Four Stages of Pressure Ulcers
The National Pressure Injury Advisory Panel (NPIAP) classifies pressure injuries into four stages, plus two additional categories for wounds that cannot be staged.
Stage 1: Non-Blanchable Redness
The skin is intact but shows persistent redness that does not turn white (blanch) when pressed. The area may feel warmer, cooler, firmer, or softer than surrounding tissue. In people with darker skin tones, the discoloration may appear blue or purple rather than red.
Stage 1 is a warning sign. The underlying tissue is stressed but not yet damaged. Intervention at this stage can prevent progression.
Stage 2: Partial Thickness Skin Loss
The outer layers of skin (epidermis and/or dermis) are lost. The wound appears as a shallow open ulcer with a pink or red wound bed, or as an intact or ruptured fluid-filled blister. There is no slough (dead tissue) or bruising present.
Stage 2 wounds are painful and require proper wound care to prevent infection and progression.
Stage 3: Full Thickness Skin Loss
All layers of skin are lost, and the wound extends into the subcutaneous fat (the tissue beneath the skin). Slough may be present. Bone, tendon, and muscle are not visible or directly palpable. The depth of a Stage 3 ulcer varies by location — on the heel or bridge of the nose, these wounds may be shallow; in areas with significant fat tissue, they can be quite deep.
Stage 3 ulcers require specialist wound care and may take months to heal.
Stage 4: Full Thickness Tissue Loss
The wound extends to muscle, bone, or tendon — which may be visible or directly palpable. Slough and eschar (dead black tissue) may be present. These wounds frequently involve infection of the underlying bone (osteomyelitis) and carry significant risk of sepsis.
Stage 4 ulcers are life-threatening and require urgent specialist care, often including surgical intervention.
Unstageable Pressure Injuries
When the wound base is covered by slough or eschar, the true depth cannot be determined. The wound is classified as unstageable until the dead tissue is removed and the wound can be properly assessed.
Deep Tissue Pressure Injury (DTPI)
A persistent area of non-blanchable deep red, maroon, or purple discoloration, or an epidermal separation revealing a dark wound bed. These injuries may be more severe than they appear on the surface and can rapidly evolve to a higher stage.
Where Pressure Ulcers Most Commonly Develop
Any bony prominence is at risk. The most common sites are:
- Sacrum and coccyx (tailbone area) — most common
- Heels — especially in bedridden patients
- Hips (greater trochanter)
- Ankles and elbows
- Shoulder blades
- Back of the head (in patients who cannot reposition)
Treatment by Stage
Stage 1 Treatment
The goal is to relieve pressure immediately and protect the skin. Reposition the patient every two hours. Use pressure-redistributing mattresses, overlays, or cushions. Apply a thin protective film dressing or moisture barrier cream to the area. Assess and optimize nutrition.
Stage 2 Treatment
Clean the wound gently with saline. Apply a moisture-retentive dressing — hydrocolloids, foam dressings, or transparent films work well for stage 2 wounds. Continue aggressive offloading. Do not massage reddened areas. Monitor closely for signs of infection.
Stage 3 Treatment
Stage 3 wounds typically require specialist wound care. Treatment includes debridement of slough, selection of advanced dressings (alginate, foam, or negative pressure therapy), nutritional optimization (protein intake is especially critical), and aggressive offloading. Infection must be identified and treated promptly.
Stage 4 Treatment
Stage 4 ulcers almost always require a wound care specialist and frequently a surgeon. Surgical debridement removes dead tissue efficiently. Osteomyelitis requires prolonged antibiotic therapy (6+ weeks) and sometimes surgical removal of infected bone. Negative pressure wound therapy and biological skin substitutes are commonly used. Skin flap surgery may ultimately be needed to close the wound.

Nutrition is as important as wound care for pressure ulcer healing. Patients with pressure ulcers have dramatically increased protein needs — typically 1.25–1.5 grams of protein per kilogram of body weight daily. Ask your care team for a nutritional assessment.
How to Prevent Pressure Ulcers
Prevention is far more effective — and less costly — than treatment.
Reposition frequently: Bedridden patients should be repositioned at least every two hours. Wheelchair users should shift weight every 15–30 minutes.
Use pressure-redistributing surfaces: Specialized mattresses, overlays, and wheelchair cushions distribute weight more evenly. They do not replace repositioning but significantly reduce risk.
Keep skin clean and dry: Moisture from incontinence dramatically increases pressure ulcer risk. Use moisture barrier products and address incontinence promptly.
Optimize nutrition: Malnourished patients have much higher pressure ulcer rates. Ensure adequate protein, calories, vitamin C, and zinc.
Inspect skin daily: Check all bony prominences daily. Early detection of Stage 1 changes allows intervention before the wound progresses.
Avoid friction and shear: Use proper lifting and transfer techniques. Do not drag patients across bed surfaces.
